Wellness Program
The University of èƵ partners with Personify Health to provide UA employees and
their spouses and Financially Interdependent Partners (FIPS) with a workplace Wellness
Program. The Wellness Program is a points based system that encourages healthy behaviors.
Those that complete the program each year will earn a wellness rebate of $600 the
following plan year! Review this page to learn more about the Wellness Program and
rebate.
Wellness Program Basics
- Benefit-eligible employees working a minimum of 20 hours per week and are enrolled in a UA Choice Medical Plan
- Spouses/Financially Interdependent Partners (FIPs) of employees as long as they are enrolled in a UA Choice Medical Plan
Temporary Employees are not eligible (visit the temporary employee webpage for eligible benefits)
- There are no biweekly premiums for the Wellness Program
- Employees who participate in the Wellness Program must be enrolled in a UA Choice Medical Plan and pay biweekly premiums on their medical plan

When logging in for the first time, be sure to use the employee's UA ID with a trailing "SP" (i.e. 30012345SP).

Wellness Program Details
8 Points Total
A total of eight (8) points are required including required activities. Spouses/FIPs
must earn their own points. All points must be earned on or before June 30 each plan
year.
2 Priority (Required) Activities
- Health Check Survey (online)
- Click on "Health" (the heart icon) in the ribbon at the top and click "Take Survey" under "Health Check"
- Biometric Screening (in-clinic, at your doctor, or mail-order kit)
- Click on "Benefits" (the hand icon) in the ribbon at the top > LabCorp > Start Now > SSO to LabCorp landing page > select screening option
6 Additional Activities
Earn an additional 6 points to qualify for the rebate. More information on earning points is available by using the "Earning Points" button below.
Once employees are logged into the platform, there is a “Rewards” drop down in the upper right. This will show the number of days remaining in the plan year along with the total points earned so far.
Because the Health Check Survey and the Biometric Screening are required activities, these must be completed before any “locked” points are officially rewarded. Employees will be able to see “locked” points, but they will not be officially added to the point total until the required activities are completed.
Must be Enrolled on UA Choice Plan
- Must be enrolled in respective UA Choice medical, dental and/or vision plan to earn points for those visits
- No points awarded for visits against insurance coverage on a non-UA Choice plan
Rebate - Employee Requirements
To be eligible for the rebate, employees must be able to answer "yes" to all of the following questions each November when the rebate is rewarded.
- I have completed the wellness program - including all required activities and additional points.
- I am a current, benefit-eligible employee during the pay period that the rebate is being awarded.
- I am enrolled in a medical plan (Premium, Basic, or HDHP) during the pay period that the rebate is being awarded.
Rebate - Spouse/Financially Interdependent Partner (FIP) Requirements
To be eligible for the rebate, spouses/FIPs must be able to answer "yes" to all of the following questions each November when the rebate is rewarded.
- I have completed the wellness program - including all required activities and additional points.
- My spouse/FIP is a current, benefit-eligible employee during the pay period that the rebate is being awarded.
- I am enrolled in a medical plan (Premium, Basic, or HDHP) with my spouse/FIP during the pay period that the rebate is being awarded.
Rebate - Enrolled in Dental/Vision Only?
Employees who are not enrolled in a medical plan (Premium, Basic, or HDHP) are not eligible to participate in the program or receive a rebate.
Earn 12 Points
Earn 12+ points and be entered to win a gift card of up to $500 at select retailers.
First Day of Coverage in the Wellness Program
- May be a delay in reporting between UA > Premera > Personify Health
- Allow for 4 weeks after employee start date before logging into the Wellness Plan for the first time
End of the Pay Period
- Coverage will end at the end of the pay period in which an employee (1) separates from the University or (2) ends coverage due to a life event
